- No cancellation fee for appointments cancelled 24 hours in advance
- $25 fee for cancellations less than 24 hours in advance.

Insurance
- Level 11 Nutrition is in the process of establishing with insurnace companies for direct billing.
- In the meantime, you can submit a reimbursement form to your insurance if your plan covers dietitian appointments.
